He was predeceased by: his parents, Bonita Slattery (Mack) and Lester Slattery. He is survived by: his wife Anne; his sons, Matthew (Mary Kay) of Two Rivers, WI and Brian (DeAnne) of Carmel, Indiana.; his grandchildren, Erin (Alex Gleichner), Colman, Molly (Jesse Silcox), Grace Perryman (Matt) and Abigehl; his stepgrandchildren, Aiden and Lexi; his sibling's children, Patrick, James Jr. Slattery and Mary Slattery; his brother-in-law Donald Fogltanz, Sr.; and his sibling's children, Donald, Jr. and Anne Louise Senglaub.
In lieu of flowers, a memorial will be established in Mack's name.
