He is survived by: his daughters, Heidi Seibak (Gary) and Pamela Kozinski (David); his grandchildren, Patrick McGowan, Anthony Kozinski (Rachael), Jeremy McGowan, Brandon Kozinski (Caitlyn), Jacob Ferguson, Alyssa Seibak, Kaile Seibak (Ryan) and Joshua Ferguson; his great grandchildren, Luca, Dekker, Bowen and Ezra Kozinski; his siblings, Vivian Klitzka of Fox Lake, IL, Dorothy Klitzka of De Pere, Carol Leslie of Mattoon, Marilyn Maus of Mattoon, Beatrice Birk (Robert) of Plantation, FL, Jayne Moore (Thomas) of Glendale, AZ and Jeff Leslie (Ruza) of Queen Creek, AZ; and the mother of his children Karen.
