He was predeceased by: his siblings, LaVon, Roger (late Donna) and Jerry (late Lyla). He is survived by: his wife Lisa; his sister Anita Bontempo (Rich); his brothers-in-law, Stephen (late Linda), Christopher (Kathy) and Gary Sumiejski (Deneen); and also Gerald Sumiejski. He is also survived by nieces, nephews, other relatives and friends.

In lieu of flowers, memorials to the charity of choice in Dan's memory would be appreciated.
