She is survived by: her children, Tammy Madsen (Dennis), Justin Polakowski (Nicci), Cindy Schmidt (Marc), David Wacholtz (Laurie) and Dawn Provost; her grandchildren, Charlene Sage, Kyle (Tanya) of Justin, Polakowski Jr., Shoshauna Daniel (Christopher), Sierra Schmidt, Lukas Schmidt (Jordan), Logan Wacholtz (Ashley), Seth Wacholtz, Zachary Wacholtz, Michael Provost Jr. and Katana Provost; her great grandchildren, Austin, Zane, Chloe Sage, Remington King, Waylon Braley, Loretta Schmidt, Henrik Daniel, Hunter Wacholtz and Mason Christensen; her niece Neveah Hoffman; her nephews, Brian Hoffman (Shelly), Tyler Hoffman, Luke Hoffman (Megan), Jordan Hoffman, Joel Hoffman, Gilbert Hoffman Jr. (Bethanne), Tommy Hoffman, Macquire Hoffman, Devin Hoffman and Jackson Fay (Kathleen Kohler); her brothers, Gilbert Hoffman (Karen) and Jerry Hoffman; her sister-in-law Cathy Nikolai; and her close friend Cheri Asinas.
