He was predeceased by: his parents, Donald Fork and Nancy Fork (McLaughlin); and his sister Constance Smith. He is survived by: his wife Edith Fork (Glover); his children, Megan Fork (Michael Lyle) of West Chester, PA and Brian Forth (Emily); his grandson Julian of Madison, WI.; and his siblings, Rolly Grover of Grand Blanc, MI, Joyce Hartman of Chesterfield, VA and Gail Demko.
