He was predeceased by: his parents, Elmer Cook and Dorothy Cook; his sister Carol Sloggy; his brother-in-law Wayne Sloggy; his nieces, Linda Millard, Victoria Flickner and Christina Angell; and his great-nephews, Daniel Flickner, Justin Angell and Ryan Angell.

He is survived by: his sibling's children, Matt Flickner (Barb) of Portage and Tammy Benson (Scott) of Montello; and his nephew-in-law John Millard of Endeavor. He is also survived by great nieces/ nephews, great- great nieces/ nephews and great-great-great nieces/ nephews.

In Lieu of flowers, monetary donations to the family to help lay him to rest would be greatly appreciated.
