He was predeceased by: his parents, Norbert Hoffman and Florence Hoffman; his siblings, Neal Hoffman (Arlyne), Georgene Schill (Gordon) and Jane Kundinger; and his brother-in-law Joe Perrizo.
He is survived by: his sisters, Sally Duerbeck (Warren) and Judy Perrizo; his brother-in-law Joe Kundinger; and his close friend Katie. He is also survived by his nieces, nephews, other relatives, and dear friends.
In lieu of flowers, memorials in Dale's memory may be directed to St. Andrew's Church Restoration Fund.
