She was predeceased by : her grandson Timothy Jones; her grandparents, John, Margaret Fleischmann (Mullen), Joseph and Natalie Kaczkowski (Oman); and her mother-in-law Leone Benner.
She is survived by : her former life partner Michael Dake; her husband Mark Gerrits; her children, Josephine Kaczkowski Dake of Seymour, Kylie Kaczkowski Dake (Brennon) of Seymour and Maxwell Dake of Seymour; her birth mother Mary Ellis (Brian); her siblings, Jennifer Ellis (Ryan) and Andrew J. Kaczkowski (Lourdes); her siblings-in-law, Terry Gerrits (Marlene), Paula Weyers (Steve), Colleen Poquette (Richard), Oneida and Jolene Waldburger (Mark); her niece Mariko Kaczkowski; her nephews, Vaughan of Chicago and Cyrus Kaczkowski of Chicago; the father of her children Michael Dake; and her friends, Polly Lund, Tammy McHugh, Paula Weyers, Tammy Van Eperen, Andy and Lourdes Kaczkowski. She is also survived by other relatives and friends.
