With solemn hearts, we announce the passing of Patrick Thomas Bohm in Shawano, Wisconsin, born in Green Bay, Wisconsin, whose presence will be deeply missed, having departed on October 11, 2025 at the age of 23. He was predeceased by: his sister Elizabeth; and his grandfather Pat "Papa Pat" Martin. He is survived by: his parents, Steve Bohm and Molly Bohm (Martin); his daughter Oaklynn; his siblings, Brittney Dillenburg (Andrew), Jennifer Knaup (Kraig), Kylie Bohm and Isaiah Bohm; his siblings' children, Brynlee, Charles, Allison, Mya, Chloe and Kaden; the mother of his child Emily Schertz; his grandparents, Arlene Martin, Roger and Nancy Bohm; his close friend Logan Duchac; his aunts and uncles, Michael, Tanya Martin, Jeff, Annette Bohm, Mike and Heather Bohm; Laurie Johnson, Emilee Bystol (Ben), Erik Johnson (Bella) and Tom Johnson. He is also survived by numerous cousins and friends.
Visitation will be held on Friday, October 17th 2025 from 4:00 PM to 7:00 PM and on Saturday, October 18th 2025 from 9:30 AM to 11:00 AM at the Swedberg Funeral Home-Shawano (518 W 5th St, Shawano, WI 54166). A funeral service will be held on Saturday, October 18th 2025 at 11:00 AM at the same location.
Memorials may be directed to Patrick's family for Patrick's daughter, Oaklynn.
