Cook, Jr.; her children, Christopher Cook (Ashley) of Dubuque, IA, Mitchell Cook (JoAnn) of Cuba City, WI, Matthew Cook (Kasha) of Shullsburg, WI, Alyson Cook and Hope Cook of Shullsburg, WI.; her grandchildren, Desmond, Lilia, Norah, Charles and Thomas Cook; her siblings, Lori Stietz (Kevin) of Apple River, IL, Timothy Harwick (Deb) of Warren, IL, Kevin Harwick (Reva) of Warren, IL, Steven Harwick (Tanya) of Freeport, IL and Tammy Renz (Mike) of Warren, IL; her father-in-law Robert C. Cook, Sr. of Warren, IL.; her siblings-in-law, Kathy McPherson of Lakeville, MN, Cara Beth Kisting of Cuba City, WI, Justin Cook (Brenda) of Shullsburg, Nathan Cook (Christa) of Apple River, IL and Caleb Cook (Shawna) of Warren, IL; and her caregiver Madison. She is also survived by many nieces and nephews.
