He is survived by: his wife Mackensey Gervasi-Lewis; his children, Safyra Gervasi, Penelope Lewis and Crosby Lewis; his mother Patricia Sorlien; his stepparents, Mark Sorlien and Connie Lewis; his siblings, Nancy DeCosta (Dennis), Robert Sorlien (Sara), Joseph Sorlien and Valerie Rosser (Tim); his niece Olivia DeCosta; his nephews, Jack DeCosta and Connor Rosser; and his in-laws, Teresa, Tadd Smejkal, Tanner Boehnen, Dale, Melissa Gervasi, Dennis, Deanna Gervasi, Halle Gervasi, Hannah Gervasi, Char and Wayne Forler.

Visitation will be held on Thursday, October 22nd 2026 from 4:00 PM to 7:00 PM and on Friday, October 23rd 2026 from 9:00 AM to 11:00 AM at the Gunderson Funeral and Cremation Care - Stoughton (1358 US-51, Stoughton, WI 53589). A funeral service will be held on Friday, October 23rd 2026 from 11:00 AM to 12:00 PM at the same location. A graveside service will be held on Friday, October 23rd 2026 from 12:30 PM to 1:00 PM.

In lieu of flowers, donations may be made directly to the family to establish an education fund for Patrick's children.
