She is survived by: her parents, Larry Duellman and Pamela Duellman (Phillips); her husband James Jacob; her children, Daniel, Leroy Jacob and Dakota Merriam; her siblings, Jennifer Duellman and Phillip Duellman (Sara); and her parents-in-law, Gene and Rose Ann Jacob. She is also survived by numerous other relatives and friends.
