He was predeceased by: his parents, Robert Middlestead and Florence Middlestead; and his brother Travis Middlestead. He is survived by: his wife Susan Middlestead; and his children, Jeff (Shari), James, Shari, Debbie, Mark and Robert. He is also survived by 9 grandchildren, 7 great-grandchildren, other relatives and friends.

In lieu of flowers, memorials to the American Legion Department of Wisconsin would be appreciated.
