He was predeceased by: his parents, Helen Weber Smith and Sherman Edward Smith; his siblings, Russel Smith, Janet Briggs (David), Natalie "Pat" Mickesh (Joe) and Carol Broadway (Bob); and his sister-in-law Rose Broadway.

He is survived by: his wife Connie; his children, Jennifer (Gary Kraft) of Wauwatosa and Thomas Smith (Donna) of West Bend; his grandchildren, Hailey, Evan Smith, Ben, Rachel and Aaron Kraft; and his brothers-in-law, Gary Norton (La Crosse) and Pat Fleming (Madison). He is also survived by nieces and nephews, and other relatives and friends.

In lieu of flowers, memorials can be made in Tom's name to St. Mary's Immaculate Conception Parish ~ West Bend or your local food pantry.
