He was predeceased by: his parents, Florian Formiller and Reemona Formiller (Kohnke); his brother Louis Formiller; his nephew Michael Formiller; his mother-in-law Deloris Domke (David "Porky"); and his brother-in-law David C. Domke.
He is survived by: his wife Teresa Formiller; his children, Tyler Formiller (Meredith Oakley) and Heather Willett (Adrian); his grandson Olin; his sister Sanda Gollnick (Steve); his sisters-in-law, Marsha Formiller and Penny Besaw; his nieces and nephews, Jeremy Formiller (Amanda), Jeffrey Ellman (Linda), Jacqueline Pluim and Jeanette Oliviera (Casper); and his friend Lisa Bouras.
Visitation will be held on Monday, June 30th 2025 from 11:00 AM to 1:00 PM at the Kwiatkowski Funeral Home (425 Jefferson Ave, Omro, WI 54963). A funeral service will be held on Monday, June 30th 2025 at 1:00 PM at the same location. A burial will be held on Monday, June 30th 2025 at 2:00 PM at the Omro Cemetery (1600 Waukau Rd, Omro, WI 54963).
