He was predeceased by: his parents, Florian Formiller and Reemona Formiller (Kohnke); his brother Louis Formiller; his nephew Michael Formiller; his mother-in-law Doloris Domke (David "Porky"); and his brother-in-law David C. Domke.
He is survived by: his wife Teresa Formiller; his children, Tyler Formiller (Meredith Oakley) and Heather Willett (Adrian); his grandson Olin; his sister Sanda Gollnick (Steve); his sisters-in-law, Marsha Formiller and Penny Besaw; his sibling's children, Jeremy Formiller (Amanda), Jeffrey Ellman (Linda), Jake Domke, Steven Duke (Domke), Scott Besaw, Jacqueline Pluim, Jeanette Oliviera (Casper) and Melissa Gollmar; and his friend Lisa Bouras.
