He is survived by: his siblings, Danny Cook and Debbie Shrewsberry; his half-sisters, Terri Blake, Sheila Brown and Pam Murphy; and his children, Tammy Crawford, Tommy Cook, Gary Cook, Lori Buis, Heather Cook and Conner Cook. He is also survived by grandchildren and great-grandchildren.
