He was predeceased by : his parent Patty S. Ballard; his grandparents, James D. Ballard Sr. and Betty M. Ballard; his siblings, Crystal L. Horton, Steven R. Horton and Michael S. Horton; his uncles, James D. Ballard Jr. and Carl P. Ballard (late Tammy R. Cheek); and his cousin Eric D. Pack.
He is survived by : his wife Shelly; his parent Patty Clyburn (Michelle Ballard) of Beckley; his children, Corey Cheek (Karyssa) of Crab Orchard, Monica Ballard (Jae) of Beckley and David Milam, Jr. of Beckley; his grandchildren, Macie B. Cheek, Chace L. Cheek, MacKenzie L. Terrell and Gracie M. Terrell; his aunts, Daisy Mullins (Charles) and JoAnn Pack; and his siblings, Jay Legursky (Desiree), Tina Daniels (Bo) and Lena Pendergrass (Danny). He is also survived by many other uncles, aunts, cousins, nephews, nieces, and in-laws.
In lieu of flowers, donations may be sent to Mark's family to help with medical and funeral expenses.
