She is survived by: her husband Chad; her sons, Hunter Scott and Eli Benjamin; her stepsons, Dakota of Charleston, WV and Addison of Charleston, WV; her parents, Paul "Scotty" Cook and Mildred Cook of Bim, WV; her grandmother Lily Coley of Bim; her great-grandparent Holcomb of Florida; and her aunts and uncles, Melissa Gay (David) of Peytona, Michelle Chambers of Van, Roger Cook (Audra) of Melina, GA and Albert Coley (Kim) of Hewett. She is also survived by great aunts, uncles, nieces, nephews and friends.
