He is survived by: his brother Harold "Bernie" Cook (Caroline); his nephew Steven; his sister-in-law Linda Kaye Cook; his nephew Brian Cook (Sandy); his great-nephews, Jalen and Noah Cook; his nephew Scotty Cook (Velma); his niece Roxanne Mayhorn (Randy); his daughter Brittni Evans (Eric); and his granddaughter Carmen.
