He was predeceased by : his parents, Hobert Koontz and Sadie Koontz (Blizzard); his daughter Sarah Marie Koontz; and his siblings, Jesse, Roger Koontz and Barbara Koontz.

He is survived by : his wife Judith Jean Koontz (Joseph); his children, Dolly Rexrode (Bill) of Sugar Grove, WV and Jackie L. Koontz, Jr. (Tiffany); his siblings, Phyllis Heavner (Jerry) of Franklin, WV, James Koontz (Glenna) of Brandywine, WV and Bob Koontz (Delores); his granddaughters, Krista, Casey and Hannah; and his great grandchildren, Chase, Travis, Karli and Kipton. He is also survived by his children, grandchildren abd great- grandchildren.
