He was predeceased by: his father David Smith. He is survived by: his mother Patty Lewis of Clarksburg; his children, Halie Hardy (David), Devon Lewis, Alayna Lewis and Alexis O'Dell (Dallion Kyrie Lewis); his brother Brandon Kyle Lewis; his grandchildren, Braxton, Grayson and Zaydyn; his aunts, Diane Jackson (Kenny), Cheryl Beatty (Gary) and Donna Forge (Cleveland); and his uncle Ricky Lewis.

In lieu of flowers, donations can be made to his Care Fund.
