He was predeceased by: his parents, Reverend Mural Lynn and Arbutas Underwood; and his children, John Underwood, Jr., Michelle and Aleana Kay Underwood.

He is survived by: his wife Angela Underwood of Tyler Mountain; his children, Delilah Jean Underwood of Tyler Mountain, Sara May of Tyler Mountain, Walter Aiden Dills of Tyler Mountain, Tiffany Sue of Tyler Mountain, Bobby Haynes of Tyler Mountain and Crystal Lynn of Tyler Mountain; his grandchildren, Daniel William Skidmore of Charleston, Jasmine of Charleston and Skylar Auber of Charleston; and his nephews, David Underwood of Charleston, Charlie Fields (Becky Funk) of Sissonville, Timmy Fields (Debbie Fields) of Sissonville, Frank and Sandy Thompson.
