He is survived by: his mother Mildred Annabelle Lambert Cook; his wife Mary Leanette Lewis; his son Joshua Cook (Jennifer) of Morgantown; his grandchildren, Jack of Elkins, Ben, Nick and Julia of Morgantown; and his stepchildren, Jeannie Louk (Chris) of Murfreesboro, TN, Oliver Williams (Melody) of Woodberry, TN and Amy Williams (Andrew). He is also survived by ten step grandchildren.
