He was predeceased by : his parents, Steve Slazo and Julie Slazo; his former life partner Peggy Slazo; and his brothers, Billy Slazo and Richard Fowler.

He is survived by : his wife Wilda Mae Slazo; his siblings, Bobby Fowler and Mazzie Hall; his daughter Cindy Clay (Wayne); his grandchildren, Trace Clay, Caitlin Smith, Tori Scaggs and Britt Comer; and his stepchildren, Sherri Smith (Don) and Ken Scaggs (Rose). He is also survived by nieces, nephews, family, and friends.
