He was predeceased by: his parents, John R. Lewis and Opal Hampton Lewis. He is survived by: his wife Shirley Jean Dyke Lewis; his children, Thomas Michael Lewis (Karen) of Pittsburgh, PA and Gena L. Lewis of Phoenix, AZ; and his sister Juanita Lewis Frame of Buckhannon, WV.
Visitation was held on Saturday, September 19th 2026 from 11:30 AM to 1:00 PM at the Chapman Funeral Home (3941 Teays Valley Road, Hurricane, WV 25526). A funeral service was held on Saturday, September 19th 2026 at 1:00 PM at the same location.
