He was predeceased by: his parents, Charles Ross Lewis and Opal Marie Randolph; his wife Minetta J. White; and his son-in-law Jefferey V. Zickefoose. He is survived by: his children, Charlotte A. Moses (Michael) of Lost Creek, Marsha K. Templin (Steven) of Lancaster, OH, Lynelle M. Zickefoose of Buckhannon, Wayne S. Lewis of Johnstown and Michelle R. Lewis of Winchester, VA.; his grandchildren, Adam L. Kwiatkowski (Christa), Kara M. Winnell (Nate), Judd S. Templin (Jessica), Clayton L Templin (Heather), Brock A. Templin (Megan), Parker J. Templin (Estefania), Emily K. McCord (Tyson) and April L Zickefoose; his great grandchildren, Kaylie Kwiatkowski, Hayden (Tiara), Dori Winnell, Jefferson, Vivian, Cecilia, Sullivan Templin, Elijah, Savannah, Sierra Templin, Scarlett, Cash Templin, Seton, Beckett, Evryn and Laurel McCord; and his brother-in-law Curtis White.
