He was predeceased by: his father Denver Jackson Perry Sr.; his grandparents, Cecil, Mary Logan Johnson, Byrd and Lillian Perry; and his close friend Arnie Branham.

He is survived by: his mother Joyce Ann Perry; his daughter Hanna; his grandson Matthew; his siblings, Kim Thomas (Donnie), Jackie Browning (Jeremy), Laurie DiMaria (Steve) and Jeffrey Perry; his pet Daisey; his nieces and nephews, Kayla, Tyler, Donnie Hall, Leanne Lively, Tabetha Vance, Natalie Ellis, Steven and Gabby DiMaria; and his great-nieces and great-nephews, Aiden Jones, Brooklynn, Landon Taylor, Hailey Ann Vance, Kingston Clark-Hairston, Anthony Jackson Chambers, Paisley Joann Hall, McKenzie Fay and Dom.

In lieu of flowers, the family request donations be made to the funeral home to help pay for the expenses.

“I had the pleasure of getting to know my cousin Jack via facebook over the last year or so. I was going through a hard time and he would always check up on me and make sure I was doing ok. He would always offer the kindest words and really helped me get through my divorce. I know he struggled with his own troubles and I will never forget how he still made sure I smiled everyday. I am so sad to hear of his passing. I will miss our fb games and convos and your silly jokes. May you rest in peace.”
