He was predeceased by: his parents, Roscoe Q. Cook and Iona Mae Cook (Rohrbaugh); his wife Nancy E. Cook (Cornell); and his brothers, Kenneth Leon Cook and Eugene Allen Cook.
He is survived by: his son David Donald Cook; and his siblings, Richard W. Cook (Nancy), Carroll B. Cook (Donna), Darvin J. Cook (Phyllis) and Sharon C. Feaster (Bradley) of Maysville, WV. He is also survived by numerous nieces and nephews.
