He was predeceased by: his parents, Gwladys McCulloh (Price) and Samuel Clark McCulloh; his son Samuel Lee McCulloh; his sister Gwladys Beale (Mary Ann Greig); and his nephew Mark Beale.
He is survived by: his close friend Patricia Llewellyn; his daughter Cindy; his sister Helen McCulloh; his niece Karen Reidler (George); his nephews, Keith Greig (Brenda), Kevin Greig (Eva), Steve Beale (Kristy) and Matt Beale; his grandchildren, Angela Walsh, Samantha Elashewich and Timothy McCulloh; his great grandchildren, Noah and Sophie Walsh; and his close friend Bill Price (Peggy).
In lieu of flowers, donations can be made in Sam's memory the First Congregational Church, 103 Bowery St, Frostburg or to the Frostburg Fire Department, 75 S. Water St, Frostburg.
