He is survived by: his son Stacey Jack Cook Jr. (Shelly) of Mullens, WV.; his granddaughters, Stephanie Cook of Mullens, WV. and Samantha Cook of Mullens, WV.; and his siblings, Terrill Ann Marshall of Maryland and Danny Cook of Oklahoma.
