He is survived by: his father Buddy Church; his close friend Robin Stanislaus; his children, Alivia, Shiane and Savannah Church; his brothers, Jamie Church (Christina) of North Spring, Melvis Church (Cassie) of North Spring, Bobby Church of North Spring and Adam Church (Charlotte) of Glen Alum; his nieces and nephews, Winter, Axle, Matthew, Marcus Church, Dusty, Renee and Christian Maynard; and his aunts and uncles, Gladys Allen, Zola Fields, Carl Thomas, Jr. of Blackberry, KY, Ralph Thomas of Blackberry, KY, Delane Thomas (Pauline) of Hardy, KY and Travis Thomas of Wharncliffe, WV.. He is also survived by relatives, cousins, and friends.
