He is survived by: his wife Donna Kay Slack; his sons, Tony Slack (April) of Belle and Timothy Slack of Sissonville; his sister Linda Burton (Marton) of Chelyan; his grandchildren, Joseph H. Slack and Beth McCarthy; and his great grandchildren, Silas Rhodes, Aubrey Rhodes, Ava Slack, Jaxson Slack and Willow Slack.
