He is survived by: his wife Tammy; his children, Karrie Mutterback and Jason Cook (Tonya); his grandchildren, Hailee Jones (Ethan), Deegan Mutterback, Maleah Mutterback and Kinlee Cook; his brother Ronald Cook (Kathy); and his pet Wylie. He is also survived by nieces, nephews, relatives, and friends.
