He was predeceased by: his grandfather Cook; his wife Mary Louise Brown Cook; his parents, Eula Sparks Cook Napier and Charles Cook, Sr.; his stepfather Irvin Napier; and his sister Phyliss.

He is survived by: his sister-in-law Rose Brown of Defiance; his nieces, Alicia Moss, Sheri Walker and Marsha Haga; and his close friend George Smithberger.
