He was predeceased by: his parents, William "Hub" Lively and Mary Lively of Rainelle; and his siblings, Jack Lively, Jim Lively, Delores Fleshman and Polly Clevenger.
He is survived by: his wife Karen Lively of Rainelle; his children, Jeremy Lively (Amber) of Ashland, KY and Autumn (Andrew Kenney) of Rainelle; his grandchildren, Emma and Allison; his sister Pat Vaughan (Marvin) of Rainelle; and his pet Maggie.
In lieu of flowers, donations may be made in Jerry's memory to Sewell Valley Baptist Church.
