He is survived by: his wife Shelvy Jean Cook; his children, Wendel Cook (Tammy), John Cook (Kelly), Victoria Cook, Keith Stine (Mary Jane), Johnathan Stine (Rebecca), Paul Lively (Ali) and Parker Lively; his siblings, Jimmy Dale Cook and Susie Cook Ball; and his grandchildren, Shane Cook, Sarah Cook Spencer, Sam Cook, Kyle Cook, Cole Cook, Abby Baker, Belle Lively, Bryan Stine and Ashton Stine. He is also survived by several other great-grandchildren, nieces, nephews, and friends.
