He was predeceased by: his parents, Michael Krivak and Helena Nagy Krivak; and his sister Sandra Urbowicz. He is survived by: his wife Janice L. Krivak (Phillips); his children, Jason N. Krivak (Marcie) and Torie A. Mazurek (Michael); his grandchildren, Aubry, Alec and Andrew Krivak; and his sister Judith Welch (Robert).
