He was predeceased by: his parents, Patsy J. Sr. Forte and Hilda L. Forte (Dellefate); his grandparents, Luigi, Maria Dellefate (Farnelli), Salvatore and Rose M. Forte (Battifarano); his brother Samuel Paul (Patsy Forte, Jr.); his aunt Isabelle Ferrelli (Forte); his niece Rosie Johnson; his godfather Vincent Lopresto; and his close friend Mark Sanders.
He is survived by: his cousins, Della, Mary Jo, Vince, Ellen and Luca Rinaldi; and his friends, Robin Sanders, Anthony Mougianis, Tara Dzvonick, Ken Davis, Rick Trimmer, Jim, Kathy Freiling and David Mosti. He is also survived by nephews, niece and great nephew.
