He is survived by: his wife Patty Hazlett; his sons, Ben and Andrew Hazlett; his sister Marie Zwick (Mitch); his niece Kimberly Huffman (Luke); his nephew Keith Zwick (Jessica); his great-nephew Conner; his stepchildren, Jared Henderson (Courtney) and Kayla Henderson; his stepgrandchildren, Hadley and Holt Henderson; his mother-in-law Susi Whitecotton; his pets, Sadie and Sara; Kelsey. He is also survived by several aunts, uncles, and cousins.
