He is survived by: his significant other Suzanne; his children, Jennifer Cook (Michael) of Wheeling, WV and Matthew Cook (Bridget) of Morgantown, WV; and his siblings, John Cook, David Cook (Terri), Richard Cook (Perri), Cathy Martin, Valery and Tiger.
