He is survived by: his wife Gayle Burns; his daughter Teri Jackson (Tom); his siblings, Pat Fitzpatrick, Jeri Olsen, Jean Taylor and Jim Burns; his cousin Earl Kincheloe; his granddaughters, Nicole and Christa; his great grandchildren, Nahlaya and Arlo; and his caregiver Poso. He is also survived by many nieces, nephews, cousins, in-laws.

A celebration of life will be held on Saturday, August 2nd 2025 at 2:00 PM at the St. John's Episcopal Church (14 Lagunitas Rd, Ross, CA 94957).

Donations in Jack's memory may be made to The Michael J. Fox Foundation for Parkinson's Research.
