He was predeceased by: his father Henry Kostreba. He is survived by: his mother Darlene Kostreba (Lange); his siblings, William Kostreba (Brenda), Lori Ann Jarnot (Jeff) and Kristi Kostreba (Shawn Shumacher); his daughter Kaylee Humphries (Zane); and his granddaughters, Maizee and Emerie Humphries. He is also survived by many nieces and nephews.
