He was predeceased by : his parents, Thomas A. "Sully" Sullivan and Jeanne Lee Sullivan (Agnew); and his daughter Tatum. He is survived by : his wife Cindy; his children, Matthew (Lara), Erin, Daniel (Tiffinni), Sequoia and Sage; and his sisters, Terrie (Eric), Kathy (TC) and Patti (Terry). He is also survived by 6 grandchildren, many nieces & nephews.
