He is survived by: his wife Dina Chavez; his siblings, Harvey Slade, Ray Slade and Donna Goodman (Don); his children, Calcee Slade and Jensen Slade; and his grandchildren, Caiden, Ezekiel and Olivia. He is also survived by numerous nieces and nephews.
