He is survived by: his wife Latoria Christian Lewis of Lafayette, LA; his children, Kieara McZeal (Dwayne) of Lafayette, LA, Chad Lewis Jr., Chaseton Lewis, Chazmon Andrus of Youngsville, LA and Trinity Lewis; his siblings, Falius Lewis (Samantha), Greg Lewis (Ruby), Elizabeth Lewis (Perry) of Church Point and Gwenodlyn Guillory (Keith) of Rayne, LA; his nieces, Lakeisha Malone of Houston and Lakayla Young of New Iberia, LA; and his sister Aretha Lewis-Davis of Lafayette.
