He was predeceased by: his parents, Margaret Whitehouse (Murphy) and John Elton Whitehouse. He is survived by: his significant other Sharon Chaisson Whitehouse (Caruso) of Stoughton; his siblings, Joseph Whitehouse (Mindy) and Joan Donelan (Tom) of Wareham; his nieces and nephews, Lisa Tufts and Jamie Donelan; his significant other's children, Brian Chaisson (Jeffrey Kane) of Easthampton and Michelle Chaisson of Stoughton; and his grandchildren, Chase and Victoria Chaisson.
In lieu of flowers, donations may be made in Jack's memory to "The Micheal J Fox Foundation for Parkinson's Research" or to "Disabled Veteran's National Foundation" to help strengthen veteran programs, assistance, and long-term support.
