He was predeceased by: his mother Judy Cook. He is survived by: his father James Cook; his wife Becky Ballard; his children, Katelyn, Brett and Reyana; and his sisters, Kim Schuler (Ron) and Stacy Datema. He is also survived by many nieces, nephews, and cousins, and countless friends.
