He was predeceased by: his parents, Robert O'Toole, Sr. and Helen Costello O'Toole; his brother Dr. Robert; his sister-in-law Harriet O'Toole; his sister Mary; and his brother-in-law G. Stephen Beals.

He is survived by: his friend Karen Chhabra of Clifton Park; his children, Sean of Manhattan, Erin (Elliot DeHoyos) of Long Beach, CA and Noreen Seaphoenix of Albany; his grandchildren, Tara (Jon Marsh) of Saratoga Springs and Vik (Nilza); and his great grandchild Ethan of Fort Myers, FL. He is also survived by numerous nieces and nephews and grandnieces and grandnephews.
