She was predeceased by: her parents, Mike Forchione and Carmela Forchione (Janello); her daughter Jackie Decker; and her siblings, Prosby, John Forchione and Angie Barnett.
She is survived by: her daughter Raeann Dolph of Canton; her grandchildren, Jason Stout (Lisa), Paul Decker (Jacinta), Dominic Archual and Samantha Archual; and her great grandchildren, Lexie, Saxon, Trent Decker, Gavin and Roman Stout.
Memorial donations, in Connie's name, may be made to St. Jude Children's Research Hospital.
