He was predeceased by: his parents, Michael Kochak and Helen Kochak (Tymochko). He is survived by: his wife Sharon Kochak (Wise); his daughters, Kaitlyn Huddleston (Charles) and Emily Nielsen (Eric); his grandchildren, Elizabeth, Ethan and Braxton; and his brother Gregory Kochak (Jacqueline) of Alabama. He is also survived by nieces, nephews.
